Our Connected Lives Gateway Service works with adults to prevent, reduce and delay need by ensuring people access a wide range of information, advice and support at the right time and in the most straight forward way possible.
We are recruiting Social Workers and Occupational Therapists to support the Gateway Service to deliver this vision as part of our plans to expand our prevention and enablement workforce in line with our Connected Lives practice model for adult social care.
The new Gateway Service strives to provide the warmest welcome for everyone. We have listened to our residents and staff and developed a new Gateway Service with prevention and enablement being at heart of everything we do. To us, enablement doesn’t just mean enabling-style homecare or bed-based commissioned enablement services, in Hertfordshire enablement is an approach and culture, designed to support people’s wellbeing in a truly person-centred way.

We are extending the time that workerscan spend supporting people to take positive risks and live the life they want through an enhanced enablement offer. Successful enablement does not always mean no formal care at its conclusion.

We are expanding our housing and enablement teams to make sure that people get better access to specialist support to realise their potential sooner.

We are increasing the professional expertise within the Gateway Service so residents can benefit from the unique skills and knowledge that Social Workers and Occupational Therapists can bring to enable them to achieve better outcomes sooner and prevent situations developing into crises.

We are investing significantly in prevention because we know that having a long-term plan will help us to meet increasing demands and will in turn create more fulfilling roles, manageable caseloads and improved staff wellbeing.
