Older people

The team works with adults over 65 with age-related needs or long term health conditions. They also work with carers looking after older people or adults with mental health or sensory needs.

Sensory services

A countywide service for adults and children in Hertfordshire with a sensory disability who may also have additional support needs to do with learning or physical disabilities, or a mental illness.

Hospital teams

Responsible for complex and vulnerable patients who need support to get home asap. We aim to help people live independently and prevent future need.

Adult safeguarding

Working with key agencies to ensure adults at risk of abuse or neglect can live, work, and be cared for in an environment free from abuse, harassment, violence or aggression.

Disability teams

Work with adults over 25 whose primary support need is because of their learning and / or physical disability. We also work with adults with autism and drug and alcohol related needs.

Community learning disability nurses

An award winning service recognised regionally and nationally as a leader for innovation and good practice. We work with NHS England on a number of health targets and have created additional nursing posts to reduce health inequalities.

Day Services

The activities we provide focus on independence, life skills and wellbeing for people with disabilities, dementia and age-related conditions. Our staff receive specialist training to deliver an excellent service.

Supported Living

Our supported living services provide up to 24-hour support for people with learning and/or physical disabilities and mental health needs. We help people to live independently in their own homes and communities.

Herts Equipment Service

From bath boards to hospital beds, this service provides equipment to disabled and vulnerable people to help them stay independent in their own homes.

Home Improvement Agency

A home adaptation service for disabled adults and children. Examples of adaptions include stairlifts, level-access showers and extensions. The service also helps with grant applications, design specification and managing contractors.

0-25 Together

Our 0-25 Together team work with young people with disabilities and their families.

Social Care Access Service

This service provides the initial gateway into adult social care, including assessment, brokerage and occupational therapy to help people live independently.

Step2Skills (previously Herts Adult and Family Learning Service)

The service runs a wide range of courses and programmes. We also help people with learning, physical or sensory disabilities or autism to find and stay in work.
