Community Learning Disability Nurse

As a community nurse, you'll work alongside paid and family carers to support people with learning disabilities to access mainstream health services.

You'll support clinicians to make reasonable adjustments.

Your skills in adapting and creating the right approach for the individual will help bridge gaps and reduce the risk of avoidable health conditions.  

Acute Liaison Nurse

You'll be working within the general hospital, offering learning disability expertise to reduce the risks of poorer outcomes for people with learning disabilities.

You'll work with people admitted into hospital for both emergency and planned admissions.

 


Band 5 responsibilities

  • Work closely with nurses and other professionals on initiatives that improve the likelihood of equitable healthcare from generic health services for people with learning disabilities.
  • Carry a clinical caseload of people with learning disabilities referred to the nursing service working in a multidisciplinary way with health and social care colleagues.
  • Be part of the nursing triage system, assessing risk and implementing nursing actions to mitigate risks.
  • Participate in regular reflective clinical supervision to support the development and maintenance of high practice standards, along with meeting revalidation requirements.
  • Provide professional advice and support to the healthcare assistants, with support from a Band 6 Community Learning Disability Nurse.
  • Assist the delivery of proactive learning initiatives using a co-production model with people with learning disabilities, those that support them and professionals on an individual or group basis, as appropriate.
  • Support student nurses through being a practice supervisor for first year after qualifying, before taking on role of practice assessor.
  • Participate in quality improvements such as the nursing audits and respond to feedback from people who use the service.
  • Increase knowledge and research and maintain a questioning and analytical approach to care.

Band 6 responsibilities

  • Identify areas where people with learning disabilities are not getting equitable healthcare from generic health services and lead the development of initiatives that improve this.
  • Carry a clinical caseload of people with learning disabilities referred to the nursing service working in collaboration with social care colleagues. As a Band 6 nurse, you will be working with more people that may have complex health issues or difficulties engaging.
  • Lead the nursing triage system, prioritising and risk assessing individuals and their situation to lead discussion and agree nursing actions with nursing teams.
  • Facilitate reflective supervision for qualified and unqualified colleagues to support the development and maintenance of high-quality practice standards, along with actively supporting revalidation of fellow nurses.
  • Provide professional advice and support to the Band 5 community learning disability nurses, with support and guidance from the Band 7 lead community nurse.
  • Develop and lead the delivery of proactive learning initiatives using a co-production model with people with learning disabilities, those that support them and professionals on an individual or group basis, as appropriate.
  • Support student nurses through being a practice assessor. Identify areas for development in student learning and support the educational audits with the university.
  • Lead on quality improvements such as nursing audits and lead on initiatives for areas of improvement as identified through feedback from people who use the service.
  • Develop systems that ensure the clinical practice of the nursing teams is based on evidenced based research and that nurses maintain a questioning and analytical approach to care.
